At Denison Middle we will provide a challenging, standards-based educational program, preparing students for success in high school and beyond.
A parent/legal guardian of the student must be present to enroll students.
To enroll in a Polk County school for the first time you must provide:
Birth Certificate or proof of age (Baptismal certificate, life insurance policy, Bible record with notarized statement, passport)
Copy of student’s social security card (optional)
Immunizations must be in compliance with the requirements of the State of Florida (an up-to-date blue card)
6th graders must have a 2nd MMR and Hepatitus B Series
7th and 8th graders must have a 2nd MMR, Tetanus Booster and Hepatitis B series
Physical within 12 months of enrollment (out of state/county is acceptable)
Two proofs of residency: (proofs must come from different categories and show parent’s name and address)
Category 1: Rent receipt or lease agreement, mortgage document,
property tax record
Category 2: Current utility bill (electric, water, gas, phone, cable)
Category 3: Voter registration document
Category 4: Proof of government benefits (Medicare, disability, food stamps, DCF correspondence)
A withdrawal form from previously attended school
To transfer from a school within the county you must provide:
A copy of the withdrawal form from the previously attended school
Two current proofs of residency (see above)
Up-to-date immunization record
Be advised that if the parent resides in Polk County, it is the residence of the parent that determine the school assignment. If a student moves during the school year, proper verification (two current proofs of residency) will need to be brought to the school before the address will be changed on the student’s record.
